Job Description

Utilizes the nursing process including: Assessment, planning, implementation, evaluation and recording of resident care.  Conducts the resident assessment and resident care plans as regulated by Health Care Financing Administration.  Practices within the scope of NC Nurse practice and Person Memorial Hospital policies/procedures standards. 

 1.        Resident assessment:  Complete thorough assessment of resident according to professional standard and hospital policy.


•         Assess resident by collecting and documenting the resident’s current health status and medical history to include physical, psychologic and social status.


•         Complete and/or review Admission forms as defined by policy and procedure.


•         Make ongoing assessment of resident’s status and respond appropriately.


•         Document all assessment in a timely and accurate fashion in compliance with applicable regulations, policies and procedures.


•         Perform ongoing assessment and revise plan of care based on new data and resident’s condition.

2.         Nursing Care:  Deliver care to residents utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.


•         Administer medications according to policy and procedure.


•         Administer IV’s according to policy and procedure.


•         Observe and document resident’s response to pertinent medications.


•         Demonstrate ability and appropriate technical skills and competencies when administering treatments and procedures in accordance with physician’s order and policy and procedure.


•         Demonstrate necessary skills and knowledge to provide care for residents according to division/unit specific competencies.

      Provide personal resident care to provide comfort and well being to resident, acknowledging physiological and psychological needs.

3.         RAI:  Complete the RAI and care plan as regulated by HCFA and State


•         Complete initial comprehensive assessment and MDS


•         Complete Raps


•         Complete quarterly, annual, significant change assessments


•         Conduct care plan rounds with nursing staff and inter disciplinary team and complete care plan as regulated


•         Transmit MDS as regulated


•         Corporate with billing department to ensure compliance with Medicare guidelines

Job Requirements

EDUCATION:

Diploma, Associate Degree or Bachelor's Degree from an accredited school of nursing.  Registered Nurse with valid/current

or temporary NC Licensure.  Basic Cardiac Life Support (BCLS)required.EXPERIENCE:Two years of experience in healthcare facility.  Preferably a skilled nursing facility.
