Acts as the primary liaison between patients, service providers, families, and guardians. Ensure patients are in a safe, appropriate environment that will provide effective treatment. Responsible for overseeing the pre-screening and admission processes.  Ensures patients meet admission criteria and ongoing compliance with CMS 13 designated ratios.  Coordinates team participation and input in the identification and development of measurable long and short term goals and barriers for timely discharges and leads Treatment Team meetings. Meets all program objectives related to CMS regulations, including but not limited to accurate, appropriate, clinical documentation, FIMs certification, Conditions of participation, etc. and meets time frames for initiation and submission of the IRF-PAI completion and submission. Coordinates with physicians, clinical staff and other hospital personnel to ensure accuracy of information included on the IRF PAI such as co-morbidities, rehabilitation diagnosis, impairment group, ICD codes and FIM scores and assures continuity of information between IRF PAI, clinical documentation and communication through program-based audits. Using discretion and independent judgment, the Case Coordinator coordinates the process of providing accurate and timely data to the appropriate entities involved in the Prospective Payment System for Medicare patients.   Will respond to third party Payor denials through formal written appeals in a timely manner as designated by CMS and hospital administration and reports denials and appeals processes in a timely manner; assures that written appeals are thorough, complete and detailed and include identification of factors that support the need for admission. Participates in attending clinical staffing, coordinating patient/family conferences; coordinating patient and family education and training related to the patient’s diagnosis, needs, and discharge; involving the patient and family in developing and monitoring goals; coordinating the patient’s rehabilitation needs; completing documentation and paperwork for the department; and promoting teamwork and cooperation within the department.  Assures privision of optimum quality patient care by serving as an assistant to the Manager/Director, assist in daily unit functions, services a reference and resource person for staff, patient and family.  Assumes responsibility for units’ activities and effectiveness of the staff and unit performance.  Respects patient confidentiality 100% of the time.

Job Requirements

Graduate from accredited school of professional nursing with a RN degree. BSN preferred.   Inpatient Rehab Facility (IPR) Patient Assessment Instrument training and certification. Experience with Utilization Review, Discharge Planning, Rehabilitation and/or Case Management experience preferred. Current licensure as RN. BLS (A.H.A.) certification required.  Rehab Facility Patient Assessment Instrument certification and Functional Independence Measurement certification required within 4 months of hire.
