Providence Health is currently looking for a PRN Case Manager for our Downtown team. The ideal candidate will have previous experience with acute care, UR and/or DCP.


Case Manager Responsibilities:

  • Facilitates continuity of care by ensuring that the needs of the patients, including observation patients are recognized and address through the coordination with other health care team members to bring about the best possible cost effective quality and clinical outcome.
  • Reviews patient admissions within one working day to determine appropriateness of hospitalization utilizing intensity of service and severity of illness criteria and continues daily reviews throughout stay assuring patient's need for acute level of care.
  • Collaborates with physician and members of interdisciplinary team to obtain input regarding discharge plan.

Job Requirements


Must possess a minimum of two yearsfull-time direct care experience and/or hospital based case management experience2 years.

A graduate from a school of nursingrequired; Bachelor degree in nursing preferred. 


Microsoft Word, Excel, PowerPoint andAccess; must have excellent planning and organizational skills; must haveexcellent written and verbal communication skills; must have satisfactory workprocessing skills; must have the ability to work independently, with little orno supervision required.

Location:Providence Health Downtown

Shift:PRN, as needed – Must be availablefor nights and weekends.
