Job Description:

The Medication Reconciliation Pharmacy Technician is responsible for working with newly admitted patients to obtain and document an accurate home medication list in an effort to help avoid medication duplications, omissions, and errors. 

Essential Job Functions:

  • Obtains accurate medication histories for patients admitted to the hospital 
  • Conducts patient and/or caregiver interviews to obtain current medication regimens including prescription and over-the-counter medications, as well as vitamin and herbal supplements for dosing, frequency, and indication for use
  • Consults with patient's pharmacy or provide(s) to investigate and verify medication discrepancies when necessary
  • Provides limited operational support to the pharmacy when needed
Hours for this position are 11 AM - 9:30 PM

Job Requirements

  • High School Diploma or GED
  • 2+years of retail pharmacy technician experience
  • Current certification with the Pharmacy Technician Certification Board (PTCB)
  • Registration with the North Carolina Board of Pharmacy (NCBOP) within 30 days of hire
  • Excellent verbal and written communication skills
