Providence Health- Downtown -  is currently looking for a Clinical Pharmacist. The ideal candidate will have previous experience at a similar size hospital or organization.  

Essential Functions:

  • Monitor medication therapy for efficacy and safety in target patient populations as part of regular patient care unit-based practice. Identify desired outcomes and contribute to the development of therapeutic plans. Intervene to prevent adverse drug events and medication errors. Ongoing collaboration with physicians, physician extenders, nurses, and other pharmacists is an essential facet of these processes
  • Augment the continuum of patient care during interdisciplinary conferences (e.g., LOS) and team-based rounds through expertise in the clinical application of medication therapy.
  • Review medication orders for appropriateness, completeness, and safety. Apply current clinical data and evidence-based methods to improve medication use in various patient populations. Assist in maintaining accurate patient medication profiles. Document activity, corresponding follow-up, and outcomes. Promote formulary system management.
  • Perform medication reconciliation audits
  • Provide straightforward, adequate, and timely drug information and clinical consultation to patient care providers throughout the organization.
  • Participate in formal medication use evaluations; apply information discovered from these assessments to promote the rational, safe, and cost-effective selection of medications.

Job Requirements

Minimum Work Experience:

Completion of an American Society of Health-System Pharmacist (ASHP) –

accredited, postgraduate residency and/or a minimum of 1-3 years’ of experience in

inpatient/acute care clinical pharmacy practice.

One or more years’ of  experience in hospital pharmacy or as hospital pharmacy student.




Minimum Education:

Graduate of a Pharm.D., Master's Degree, or PhD program with clinical pharmacy practice concentration.

Program recognized by the Accreditation Council for Pharmacy Education (ACPE). 


Required Licenses:

[South Carolina, United States] Pharmacists

Active licensure as a pharmacist in South Carolina.
