Central Carolina Hospital

Consultswith nursing staff and multidisciplinary team regularly to evaluate patient'sstatus and appropriateness of medical care, including admission, length ofstay, transfer and discharge.

Monitors patient andfamily satisfaction. Responds to questions and complaints from patients, familymembers, and payors regarding care.

Participates indischarge planning including coordinating patient transfers to other facilitiesand coordinating community resources. Provides discharge education and resourcereferrals to patients.

Performs chart review toidentify actual or potential issues with service delivery, patient outcomes andsatisfaction, compliance, cost, and reimbursement.

Job Requirements

Education: ADN or BSN

Experience: 5 years experience in Case Management that includes

clinical/professional expertise in social work, utilizationmanagement,

discharge planning, and case management. Management experience.

Licensure/Certification/Registration: Current North Carolina orCompact state

RN licensure required. BLS required upon hire.

Skills: Must have the ability to analyze statistical data, preparereports, develop

policy and procedures and provide overall direction to personneland medical

 staff related to Case Management initiatives. interpersonalskills are required

 to act as a liaison among all members of the healthcareteam. Effective

 communication and interpersonal skills.
