Job Description for RN Navigator - Oncology Services:
  1. Communicates/Collaborates with AD’s, Medical Director and members of the health care team. 
  2. As the Oncology Nurse Navigator, participates in assessing, planning, implementing and evaluating oncology health services.
  3. Reports outcomes and participates in quality improvement initiatives.
  4. Assesses and identifies high risk patients (physical, psychological and social.)
  5. Identifies medical staff and community needs for education and services.
  6. Collaborates with other educators to provide educational offerings.
  7. Identifies/implements and evaluates the education plan based on patient’s assessed need.
  8. Demonstrates appropriate and assertive methods of communication.
  9. Promotes an interactive relationship that helps communication among patient, family and nurse.
  10. Ensures’ patient’s right to choice.
  11. Manages services throughout the continuum of care by discussing plan of care.
  12. Responsible for developing and overseeing the care coordination provided to the client and is liaison between the MD, professional health care staff and others involved parties.

Job Requirements

Current RN license in State of NC, OCN preferred, BSN required.
Minimum of 2 years of Oncology Nursing Experience required.
