Assesses and evaluates assigned patients. Conducts individual, family and/or group therapy. Functions as a primary contact for treatment and discharge planning. Promotes implementation of therapeutic milieu and daily program schedule. Maintains a safe and efficient working and treatment environment per facility policies and procedures. Communicates effectively with the treatment team to ensure safe, quality care is provided to all patients.

Effectively performs as a treatment team member, takes and active leadership in the treatment team process.

Demonstrates the ability to increase awareness of existing community resources to the patient, family, and caregivers.

Demonstrates the ability to effectively communicate recommendations of the treatment team to the patient, family, caregivers and other support systems.

Effectively interviews patient, family members and other support systems to access bio-psycho-social status of patients scheduled for discharge. Formulates a discharge plan in a timely manner that matches patients with organizations, settings and services best able to meet the needs of the patient. Provides ongoing assessments during hospitalization to ensure appropriate referrals based on status at the time of discharge.

Demonstrates the ability to integrate age specific factors into the plan of care and involves patient and family into the care plan process.    

Consistently conducts and documents a bio-psycho-social assessment within 72 hours of admission.

Facilitates groups per Program Schedule.

       Effectively counsels patients and their families regarding psychosocial needs or problems.

Job Requirements

Masters Degree in Social Work required.

LCSW is required.

Licensed to practice as clinical social worker.

BLS required and provided

Hospital approved Behavioral Management Program will be provided. 


Experience providing crisis services

Experience working in the Emergency Department

Experience working on an inpatient Behavioral Health Unit

Experience using Telemedicine equipment for assessment

Knowledge of Recovery and Wellness Philosophy
