SOVAH Health - Martinsville

Performs duties for the Care Management Director.  Assist Care Managers in development of discharge plans for patients with special emphasis on Nursing Home/Rehab/Placement.  Communicates with patients, families, physicians, resources, and referrals to establish timely discharge plan appropriate to meeting specific needs of patients.  Facilitates adoptions, child protective services issues, adult protective service issues and any other concerns/requests for issues with various community resources inclusive of Department of Social Services.  Negotiates acceptance of Medicare patients/Commercial Insurance Clients/medically complex patients by skilled nursing facilities, etc.  Completes Medicaid screenings. 

Job Requirements

Education Bachelor’s Degree in Social work or related field preferred
Experience: Six months to one year in medical setting preferred 
Skills:  Ability to communicate effectively with patients, families, medical and public professionals.  Ability to work under stressful situations.  Ability to follow policies and procedures. 
Basic computer skills knowledge.  Ability to handle information in confidential manner
Typical Physical Demands: Requires full range of motion including manual finger dexterity and eye-hand coordination.  Requires standing and walking for extensive periods of time. 
Occasionally lifts and carries items weighing up to 25 pounds.  Requires corrected vision and hearing to normal range.  Requires working under stressful conditions and/or irregular hours.  Potential for exposure to communicable diseases and/or body fluids.
