Georgetown Community Hospital

Under the general direction of the Director of Case Management, provides integration, coordination and advocacy for individuals requiring emergency services in the hospital.  Performs utilization review, discharge planning and resource management in accordance with state, and federal guidelines, as well as other payer requirements following established hospital or departmental policy and procedures.

1.           Requires knowledge of Case Management policies and procedures and State and Federal regulations regarding medical necessity for admission and discharge screens.

2.           Knowledge of community resources and discharge planning, along with the technical skills necessary to demonstrate alternative care programs to patients and their families.

3.           Performs preadmission reviews on all ED patients referred for admission utilizing standard criteria (e.g. Interqual/2 MN Rule) to determine appropriate level of care for the patients.

4.           Demonstrate the ability to maintain strict patient medical, financial, and other information confidentiality at all times.

5.           Strong interpersonal skills and the ability to communicate effectively with patients, families, nursing staff, physicians, associations and community resource liaisons are inherent to this position.

6.           Demonstrates the ability to work as a team member with the ED staff, physicians and registrations staff to provide quality patient services.

7.           Demonstrates the ability to provide interventions based on age-specific criteria and departmental patient care standards.

8.     Demonstrates knowledge of Infection Control guidelines and the following of the seven safety plans: safety, security, hazardous materials and waste, emergency preparedness, life safety, medical equipment, and utility systems.

9.      Provides the information to determine the medical necessity of all admissions.  Identifies30 day readmission during ED visit, reviews last admission and potential alternative LOC setting if appropriate.

10.  Ability to manage time and perform job duties in an organized manner.

11.  Observe professional ethics in maintaining confidential information acquired concerning the personal, financial, medical or employment status of patients

12.  Performs other job duties as needed.

Job Requirements

Must be a RN in the State of KY.

License must be in good standing.

CPR required thru American Heart.

Minimum of a Clinical Ladder 1 required.
