SageWest Healthcare - Riverton

Plans, organizes and implements social work services to patients and families. Coordinates social services activities in discharge planning as required. Maintains contact with referring agencies and related people, and with community resources used for referral. Develops bio-psychosocial treatment plans and delivers individual, group and family psychotherapy. Follows the guidelines described in the Code of Conduct Ethics and Compliance Program. Participates in the Inpatient LCSW call rotation.

Job Requirements

  • Current LCSW license from the State of Wyoming
  • Prefer minimum of 2 years experience in social casework in a healthcare setting
  • Administrative and organizational skills
  • Working knowledge of state and local mental health resources
  • Maintains a professional relationship with coworkers
  • Manages interpersonal conflicts effectively
  • Accepts responsibility for daily job performance. Completes assignments in a timely manner
  • Able to establish and maintain appropriate relationships with patients, their families, co-workers, mental health and other human services professionals
  • Able to communicate and comprehend accurately and effectively in English through both written and verbal correspondence
  • Must have knowledge through training or experience in performing patient care activities for adult patients (18-64) and geriatric patients (ages 65+)
