SOVAH Health - Martinsville

As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.  Performs basic patient care activities as delegated and supervised by an RN. Assists patients with personal care and activities of daily living, including but not limited to bathing, grooming, eating, and ambulation.  Contributes to the assessment of patients by collecting data for analysis, under the direction of an RN. Reports abnormal findings or changes in physical, mental, and emotional conditions to RN.  Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.  Performs interventions including (but not limited to) specimen collection, vital sign monitoring, blood glucose measurements, monitoring of intake/output, and wound care. Documents patient care given.  Administer medication and document such activity.  Works as an advocate for the physical and emotional well-being of the patient.  Assists with keeping patient rooms and common areas stocked, clean and orderly.  Performs other duties as assigned by Supervisor or Director.

Job Requirements

Minimum Education: Current licensure as Licensed Practical Nurse (LPN) from the Virginia State Board of Nursing. 
Previous experience in acute care hospital setting preferred.
Required Skills:  Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Verbal ability to present information and ideas related to nursing theory and practice. Ability to accept responsibility and accountability to perform delegated aspects of the nursing process. Requires the cognitive ability and practical ability to provide nursing care appropriate to the patient population assigned, considering patient age and diagnosis with full implementation of the nursing process to include Assessment/Reassessment, Nursing Diagnosis, Planning, Implementation, and Evaluation.
 Required Skills/Certifications:  BLS certification – if not current, training required. PALS certification if applicable.
