Providence Health is currently looking for an experienced RN Case Manager to join our team. Candidates with hospital, nursing home, hospice or home health experience are strongly encouraged to apply. 

 

Case Manager Summary: Facilitates continuity of care by ensuring that the needs of the patients, including observation patients are recognized and address through the coordination with other health care team members to bring about the best possible cost effective quality and clinical outcome. Reviews patient admissions within one working day to determine appropriateness of hospitalization utilizing intensity of service and severity of illness criteria and continues daily reviews throughout stay assuring patient's need for acute level of care. Collaborates with physician and members of interdisciplinary team to obtain input regarding discharge plan.


Job Requirements

Qualifications/Specifications:

Must possess a minimum of two yearsfull-time direct care experience and/or hospital based case management

experience 2 years.

A graduate from a school of nursingrequired; Bachelor

degree in nursing preferred. 

Must be proficient with MicrosoftWord, Excel, PowerPoint

and Access; Must have excellentplanning and

organizational skills; Must haveexcellent written and verbal communication skills; Must have satisfactory workprocessing skills; Must have the ability to work independently, with little orno supervision required.
